It looks like stuck shed to me. It can definitely appear to come out of nowhere. I would begin dousing that foot in vitamin E oil, increase the humidity to at least 80%, and soak him twice daily in a mild iodine solution (the color of a lightly steeped black tea) to soften the skin. You can help remove it as it softens and peels. It is also possible that there is some sort of injury, but to me this looks like a shed issue. Good luck, praying for the little cherub
